Cuidados paliativos no paciente com demência avançada: uma revisão narrativa

A demência avançada está associada à profunda deterioração cognitiva e alterações físicas. Essa doença neurodegenerativa é considerada uma das principais causas de incapacidade entre a população em envelhecimento e está relacionada a uma dependência progressiva dos cuidadores. Com o aumento da população idosa e consequente carga de cuidados de demência, é preciso considerar a necessidade de serviços e cuidados paliativos como uma prioridade urgente de saúde pública. Portanto, o presente estudo teve como objetivo investigar quais são os cuidados paliativos a serem adotados no manejo do paciente com demência avançada. O método utilizado foi a Revisão de Literatura do tipo Narrativa, utilizando-se de artigos científicos, capítulos e livros publicados a partir de 2013, disponíveis nas bases de dados Scielo, PubMed e Biblioteca Virtual em Saúde. Evidenciou-se nesta pesquisa que o reconhecimento e a avaliação da demência, bem como o desenvolvimento de planos de cuidados eficazes e abrangentes são importantes para reduzir o fardo da doença. Os cuidados paliativos são apropriados em qualquer momento após o diagnóstico e incluem estabelecer o local de cuidado na última fase da vida, o gerenciamento de sintomas como dor, dispneia e disfagia, bem como cuidados com alimentação, conhecimento de agentes farmacológicos e gerenciamento psicossocial. Conclui-se que os cuidados paliativos estão relacionados à qualidade de vida ao longo da trajetória de uma doença, ajudando as pessoas com demência a viver, proporcionando conforto em vez de prolongar a vida a todo o custo.
